In consideration for the privilege of participation in Yoga classes, Health Programs or Workshops (the "Programs") offered by Yoga Recycled Inc., I hereby agree that:

1. I understand that there are inherent risks involved in any physical activity, including but not limited to Yoga. I expressly assume all risks associated with my participation in any and all such physical activity, including programs sponsored by Yoga Recycled Inc.. I know and agree that these risks attendant to physical activity and exercise include the risk of serious physical injury and even death. In exchange for being allowed to participate in, and receive instruction from Yoga Recycled Inc., I fully assume all risks associated with my participation; I knowingly waive my right to blame or bring suit against Yoga Recycled Inc.. It is my intent that this waiver be construed as broadly as legally possible and that the matters I am waiving shall include, but not be limited to, gross negligence, injuries resulting from intensive physical activity and exertion, causation or aggravation of a new or existing physical injury or medical condition, lack of warnings or inadequate warnings: lack of instructions, inadequate instructions, or my failure to follow instructions: slipping from slippery surfaces such as mats or floors, injuries while driving to and from Yoga Recycled Inc. classes, theft of any material item while at or around any Yoga Recycled Inc. class, and the like. Again, I am fully aware of and accept the risks and hazards involved, and agree to assume full responsibility for any risks, conditions, injuries or damages, known or unknown, which I might incur or aggravate as a result of my participation in the programs.
2. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the Yoga Classes, Health Programs or Workshops, and to receive prior approval to participate. I represent and warrant that I am physically fit and I have no medical condition or injury, which would prevent my full participation in the Yoga Classes, Health Programs and Workshops.
3. I, for myself and my heirs, assigns, successors, executors, administrators, and legal representatives hereby release and agree that I will not sue Yoga Recycled Inc., its Affiliates, Officers, Directors, Agents, Servants, and Employees or the landlord of any premises at which it may operate, for money damages for personal injury or property damage sustained by me during my use of presence in, and/or participation in the facilities used by Yoga Recycled Inc., equipment and Programs.
4. I, for myself and my heirs, assigns, successors, executors, administrators, and legal representatives hereby agree I will defend, indemnify and hold harmless Yoga Recycled Inc., its Affiliates, Officers, Directors, Agents, Servants and Employees or the landlord of any premises at which it may operate, from any and all claims, suits or demands by anyone arising from my use of, presence in, and/or participation in the Yoga Recycled Inc. facilities, equipment and Programs.
5. I understand that it is my continuing responsibility to inform the instructor(s) at Yoga Recycled Inc., of any previous medical conditions, injuries or surgeries prior to my first class and at such other times as I acquire information as to same. While I understand that Yoga Recycled Inc. will take any injury or medical condition which I might report into consideration in attempting to assist me in my Yoga practice, I agree that regardless of what I report to Yoga Recycled Inc. regarding prior injuries or medical conditions, there shall be no heightened duty on the part of Yoga Recycled Inc. and nothing I might report by way of prior injuries or medical condition shall abrogate or alter the express assumption of the risk and the express waivers contained herein. I agree that if I have any questions about my fitness or the appropriateness of my participation in any program sponsored by Yoga Recycled Inc. I will direct those questions to a duly qualified and licensed medical doctor before participating in any Yoga Recycled Inc. program.
6. I understand that Yoga Recycled Inc. may, in its sole discretion, and without explanation and for any reason deemed appropriate by Recycled Yoga, refuse to allow me to participate in any program.

I have read the above Release and Waiver of Liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
